Liability Waiver &
Release Form

1. Parties & Services

This Liability Waiver and Release ("Waiver") is entered into between Run Bud Mobile Canine Fitness ("Run Bud," "we," "us") and the dog owner or authorized handler signing below ("Client," "you"), in connection with mobile canine treadmill fitness sessions and related add-on services (nail trimming, potty breaks) performed at Client's home or other agreed location in Jefferson City and Mid Missouri.

2. Assumption of Risk

Client understands that physical exercise and handling of animals carry inherent risks, including but not limited to:

• Slips, falls, muscle strain, or other injury to the dog during treadmill use or handling
• Unpredictable animal behavior, including reactivity toward Run Bud staff, equipment, other animals, or persons present
• Risk of bloat (gastric dilatation-volvulus) or gastrointestinal upset associated with exercise too soon after eating or drinking
• Minor nicks or discomfort during nail trimming services, if selected
• Property contact incidental to mobile equipment setup in Client's driveway, garage, or yard

Client voluntarily assumes all such risks, known and unknown, associated with these services.

3. Release of Liability

To the fullest extent permitted under Missouri law, Client releases, waives, and discharges Run Bud, its owner, employees, and contractors from any and all claims, liabilities, or damages arising from ordinary negligence in connection with the services described above. This release does not apply to, and Run Bud remains responsible for, any injury or damage caused by gross negligence, recklessness, or willful or wanton misconduct.

4. Client Disclosures & Responsibilities

Client confirms that, to the best of their knowledge, the dog identified below:

• Is current on rabies and core vaccinations, or Client accepts responsibility for any risk arising from non-vaccination
• Has no known medical condition (heart, joint, respiratory, or otherwise) that would make treadmill exercise unsafe, unless disclosed in the notes section below
• Has not been withheld water or food in a manner inconsistent with the pre-session guidance above, and Client has read and agrees to follow the no-water-15-minutes-prior guidance
• Has no history of severe aggression toward people that has not been disclosed to Run Bud in advance

Client agrees to disclose any change in the dog's health, medication, or behavior before each session.

5. Emergency Veterinary Care

In the event of an apparent medical emergency during a session, Run Bud staff may, at their discretion, transport or arrange transport of the dog to the nearest available veterinary clinic. Client authorizes this action and agrees to be financially responsible for any resulting veterinary costs. Run Bud will make reasonable efforts to contact Client first, but emergencies may require immediate action.

6. Photo & Media Release (Optional)

Client may separately opt in below to allow Run Bud to photograph or video the dog during sessions for use in marketing materials, including the website and social media. This section is optional and is not a condition of receiving services.

7. Governing Law

This Waiver is governed by the laws of the State of Missouri. If any provision is found unenforceable, the remaining provisions remain in full effect. This Waiver does not waive any right or protection that cannot be waived under Missouri law, including claims for gross negligence, recklessness, or willful misconduct.

8. Acknowledgment

By signing below, Client confirms they have read, understood, and voluntarily agree to this Waiver, are the legal owner or authorized handler of the dog described, and are at least 18 years of age.
